Responsibilities
• RN must prep for the visit the morning of or afternoon before (Review of EMR and previous assessments)
• Travel to the members home to conduct the visit.
• Accurately document according to the provided documentation template which is a minimum of what is expected.
• Complete 2-5 visits depending on the type of visit assigned. Visits have different time allotments assigned based on what is required during the visit.
Required Qualifications
• Active RN license for NY
• Valid drivers licnese(cannot rely on public transit, they need to have a car)
• MLTC, MAP or Maximus experience
• 3+ years of care management
• UAS exp HIGHLY preferred
• Home health, discharge planning, or long term care experience preferred
• Bilingual in Spanish preferred
