When it comes to increasing the value of healthcare, case managers play a key role

As more healthcare moves towards a value-based care system, it is becoming more important than ever to improve outcomes of patients over time. Case managers educate their patients to assist them in becoming more actively involved in their own care. As a Health Affairs study on patient outcomes shows, the more engaged and active the patient is in their own care, the better the outcomes.

The role case managers play in navigating through the system creates a prime opportunity to utilize them as a transition coach. They work with patients and caregivers to help them feel comfortable working with providers, know what questions to ask and what are “red flags” they should watch for in the health of the patient. By arming the patient and caregiver with this knowledge, hospitals implementing the case manager as a transition coach have been able to decrease their readmissions and increase their patient satisfaction.

Additionally, the case manager’s role in providing discharge planning can save facilities money by decreasing the length of stay. They can help ensure that the patient has the correct medical equipment and prescriptions. Case managers also coordinate with insurance companies to ensure that claims are being paid and push back on denials as needed and assist uninsured patients in getting coverage or shortening the length of stay to reduce costs.
